Transition: Community & Nursing Home
Do you, or someone you know, live in a nursing home and want to live in your own home?
Access to Independence is able to assist you or someone you know move from a nursing home to a home in the community. We work with individuals with disabilities and various agencies in the community to help transition from nursing homes, assisted living centers, or other institutions into their own homes.
Together we help create a transition plan, apply for benefits, obtain personal care, locate housing, and acquire assistive technology.
